1. Introduction
You are reviewing and acknowledging this Telehealth Informed Consent because you are seeking healthcare services using telehealth technologies facilitated through Medical Media Partners LLC – DBA TrimToday and the trytrimtoday.com platform (collectively, the “TrimToday Platform”). This Telehealth Informed Consent supplements but does not modify or supersede our Terms of Service, Privacy Policy, or Notice of Privacy Practices.
By creating an account, starting a consultation, clicking “I consent to telehealth,” checking a related box, or using any other acceptance protocol through the TrimToday Platform, you indicate that you have reviewed the risks described herein and consent to receive telehealth services.
2. What is Telehealth?
Telehealth involves the delivery of healthcare services using electronic communications, information technology, or other means between a licensed healthcare professional in one location and a patient in another location. Telehealth may be used for diagnosis, treatment, follow-up care, prescription management, patient education, and related healthcare services.
At TrimToday, telehealth includes:
- Online medical intake forms reviewed by licensed providers
- Asynchronous (non-real-time) communications via secure messaging, email, and SMS
- Live video or phone consultations when clinically indicated
- Electronic prescription transmission to partner pharmacies
- Ongoing care coordination through your TrimToday care team
3. Possible Benefits of Telehealth
- Easier and more efficient access to healthcare services.
- Ability to receive care at convenient times without needing an in-person office visit.
- Continuity of care with ongoing support from a licensed care team.
- Faster access to medications, with overnight refrigerated shipping.
- Discretion and privacy when receiving treatment for sensitive conditions.

4. Possible Risks of Telehealth
While telehealth offers many benefits, it also carries potential risks that you should understand:
- Limited information: Information transmitted electronically may be insufficient for appropriate clinical decisions, especially compared to an in-person physical examination.
- Technology failures: Internet, mobile, or platform technology issues may delay services or cause communication problems.
- Privacy considerations: Electronic communications, despite security measures, carry greater privacy risks than in-person care. There is a small risk of unauthorized access during transmission.
- Incomplete records: Incomplete records or poor-quality data could lead to misdiagnosis, adverse drug interactions, or other clinical errors.
- Diagnostic limitations: Some conditions may not be diagnosable through telehealth and may require in-person evaluation. Diagnostic tests ordered through the TrimToday Platform are performed by third-party laboratories and may produce erroneous results.
- Compounded medications: Compounded medications used in your treatment plan are not individually FDA-approved as finished products and may differ in safety, effectiveness, and quality from commercially available FDA-approved versions. See Section 6 for additional details.
- Communication delays: Despite 24/7 availability, response times may vary based on inquiry volume, time of day, and complexity.
5. GLP-1 Medication Specific Risks
If you receive a prescription for a GLP-1 medication (such as semaglutide or tirzepatide) through TrimToday, you acknowledge the following GLP-1-specific risks and considerations:
5.1 Common Side Effects
The most common side effects of GLP-1 medications are gastrointestinal:
- Nausea (most common, especially in the first 4-8 weeks)
- Vomiting
- Constipation
- Diarrhea
- Reduced appetite
- Headache, fatigue, and dizziness
- Injection site reactions (mild redness, soreness, or swelling)
Most side effects improve over the first 4-8 weeks of treatment as your body adjusts. Your provider may adjust your dose if side effects persist or become severe.
5.2 Serious Risks
Less common but potentially serious risks include:
- Pancreatitis (severe inflammation of the pancreas) — symptoms include severe persistent abdominal pain that may radiate to the back, with or without vomiting.
- Gallbladder problems including gallstones and gallbladder inflammation.
- Kidney problems, including acute kidney injury, especially with severe vomiting or dehydration.
- Severe hypoglycemia (low blood sugar), especially when GLP-1 medications are combined with other diabetes medications such as insulin or sulfonylureas.
- Diabetic retinopathy complications in patients with type 2 diabetes.
- Allergic reactions, ranging from mild skin reactions to severe anaphylaxis.
- Gastroparesis (delayed stomach emptying), which may worsen pre-existing conditions.
5.3 Black Box Warning: Thyroid C-Cell Tumors
The U.S. Food and Drug Administration has issued a Boxed Warning for GLP-1 medications regarding the risk of thyroid C-cell tumors, including medullary thyroid carcinoma (MTC). Animal studies have shown that GLP-1 medications cause thyroid C-cell tumors. It is unknown whether GLP-1 medications cause similar tumors in humans.
You should NOT use GLP-1 medications if you or any of your family members have a history of:
- Medullary thyroid carcinoma (MTC)
- Multiple Endocrine Neoplasia syndrome type 2 (MEN 2)
Tell your provider immediately if you experience a lump or swelling in your neck, hoarseness, trouble swallowing, or shortness of breath while taking GLP-1 medication.
5.4 Additional Contraindications
GLP-1 medications are also not appropriate for individuals with:
- Personal history of pancreatitis
- Severe gastroparesis or other severe gastrointestinal disease
- Type 1 diabetes (GLP-1 medications are not insulin replacement)
- Current pregnancy, plans to become pregnant within 2 months, or breastfeeding
- Active eating disorder (anorexia nervosa, bulimia, binge eating disorder)
- Severe kidney disease or end-stage renal disease
- Severe liver disease
- Hypersensitivity to semaglutide, tirzepatide, or any ingredient in the formulation
- Active suicidal ideation or recent history of suicidality
- Age under 18
It is your responsibility to disclose all relevant medical history during your intake. Your provider may decline to prescribe GLP-1 medications if you have any of these contraindications.
6. Compounded Medication Acknowledgment
Important: The compounded semaglutide and tirzepatide medications dispensed by our partner pharmacies are not individually FDA-approved as finished products. Compounded medications are prepared by licensed 503A compounding pharmacies (Logos Pharmacy and Angel Pharmacy) under FDA-regulated compounding standards, but they have not undergone the same individual product approval process as commercial brand-name versions (Ozempic, Wegovy, Mounjaro, Zepbound).
Compounded medications may differ from the FDA-approved branded versions in:
- Specific inactive ingredients (excipients)
- Strength concentrations and dosing formats
- Stability and storage requirements
- Manufacturing processes
By accepting this consent, you acknowledge that you understand compounded medications are not separately FDA-approved, that they are prepared per-patient by licensed compounding pharmacies, and that your provider has determined a compounded medication is appropriate for your individual care.
7. Patient Acknowledgments
By accepting this Telehealth Informed Consent, you acknowledge that you:
- Have reviewed this document in its entirety and understand the risks, limitations, benefits, and contraindications of telehealth services and GLP-1 medications.
- Understand that electronic communications carry greater privacy risks than in-person care, despite the security measures we have in place.
- Recognize that your Provider may be a licensed physician, nurse practitioner, or physician assistant, depending on state regulations and clinical appropriateness.
- Acknowledge that all information you provide during intake, consultations, and follow-ups is accurate, complete, and current. You agree to promptly update your medical information if anything changes.
- Understand that there is no guarantee of prescription, and that prescribing decisions rest entirely with your Provider’s professional medical judgment based on your individual circumstances.
- Know that you have the right to withdraw consent for telehealth at any time without prejudice to your future care or any other rights you may have.
- Acknowledge that you may access your medical records through the Platform in accordance with applicable law and our Notice of Privacy Practices.
- Understand that you may choose any pharmacy or laboratory for services. If you choose a pharmacy other than our partner pharmacies (Logos or Angel), you will be responsible for arranging pickup and payment directly.
- Acknowledge that creating an account on trytrimtoday.com does NOT by itself create a doctor-patient relationship. A doctor-patient relationship is established only when a Provider reviews your intake and accepts you as a patient.
- Agree that Medical Media Partners LLC – DBA TrimToday is a third-party beneficiary of this Telehealth Informed Consent and has the right to enforce its terms.
- Consent to receiving electronic communications, including secure messages, SMS, and email, as part of your care. SMS terms are detailed in our Terms of Service.
- Understand that consultations may not be recorded; if any recording is to occur (for example, for medical record purposes), your Provider will obtain your separate consent at the time.
- Acknowledge that you will follow all storage instructions provided with your medication, including refrigeration where required, and that you are responsible for proper storage after delivery.
8. Pregnancy and Breastfeeding
GLP-1 medications are not appropriate for use during pregnancy or breastfeeding. If you are pregnant, planning to become pregnant within the next 2 months, or breastfeeding, you should NOT use GLP-1 medications.
If you become pregnant while taking a GLP-1 medication:
- Stop taking the medication immediately
- Contact your TrimToday provider via SMS at 727-228-1025
- Notify your obstetrician or primary care provider
Effective contraception should be used during treatment for individuals of childbearing potential who could become pregnant.
9. Your Right to Withdraw Consent
You have the right to withdraw your consent to receive telehealth services at any time, without prejudice to your future care. If you withdraw consent:
- Your subscription will be canceled per the terms in our Terms of Service.
- Your medical records will be retained per HIPAA and state law requirements (see our Notice of Privacy Practices).
- You may request a copy of your medical records at any time.
