What is semaglutide and how does it work?
Semaglutide is a synthetic version of GLP-1 (glucagon-like peptide-1), a hormone your body naturally produces after eating. The medication works in three primary ways: it slows the rate at which food leaves your stomach, so you feel full longer after meals. It reduces appetite signals in the brain’s hypothalamus, so you feel less hungry between meals. And it helps regulate blood sugar by stimulating insulin release when glucose levels rise.
The combined effect is that most patients naturally eat less without conscious calorie counting or restrictive dieting. Most people notice reduced food cravings within the first one to two weeks of treatment, with visible weight loss typically beginning in weeks two to four and accelerating over the following months.
Who is semaglutide for?
Semaglutide is generally appropriate for adults age 18 and older who meet medical criteria for GLP-1 weight management. This typically includes adults with a body mass index (BMI) of 27 or higher with weight-related health conditions, or BMI of 30 or higher regardless of other conditions. Your Trim Today provider will review your complete medical history during the intake process and determine whether semaglutide is appropriate for you.
Semaglutide is not appropriate for people with a personal or family history of medullary thyroid carcinoma, Multiple Endocrine Neoplasia syndrome type 2 (MEN 2), severe gastrointestinal disease, current pregnancy or active attempts to conceive, active eating disorders, history of pancreatitis, or severe kidney disease.
What dose will I take?
Most patients begin at a low starting dose (typically 0.25 mg per week) and titrate upward gradually as tolerated, often reaching maintenance doses between 1.0 mg and 2.4 mg per week. Unlike many competitors, your dose increases never increase your bill at Trim Today — one flat price covers any dose your provider prescribes for the duration of your program.
What are the side effects of semaglutide?
The most common side effects are gastrointestinal: nausea, mild constipation or diarrhea, occasional vomiting, and reduced appetite. These typically peak in the first few weeks after starting or after a dose increase, then improve as your body adjusts. Most patients tolerate the medication well, especially with gradual titration. We offer Zofran (ondansetron) as an add-on therapy to help manage nausea during the early weeks of treatment.
Less common but serious side effects can include pancreatitis, gallbladder issues, kidney problems, and severe hypoglycemia (especially when combined with other diabetes medications). Patients should contact their provider immediately if they experience severe abdominal pain, severe vomiting, or any concerning symptoms.

Tirzepatide
A newer dual-action medication that targets both GLP-1 and GIP receptors, often producing greater weight loss than semaglutide alone. Available at Trim Today as a compounded medication starting at $167 per month.

What makes tirzepatide different from semaglutide?
Both medications work by mimicking gut hormones that regulate appetite and blood sugar. The key difference is that tirzepatide activates two appetite-regulating receptors instead of one — both GLP-1 (the same target as semaglutide) and GIP (glucose-dependent insulinotropic polypeptide). This dual action tends to produce greater average weight loss in clinical trials, though individual response varies. Tirzepatide is also newer to market and typically priced higher than semaglutide.
How does tirzepatide work?
Tirzepatide’s mechanism builds on the GLP-1 effects (delayed gastric emptying, reduced appetite signals, improved blood sugar regulation) and adds GIP receptor activation, which appears to enhance insulin sensitivity and potentially boost the appetite-suppressing effects. The combined effect is that many patients experience even faster and more pronounced changes in appetite and weight compared to single-mechanism GLP-1 medications.
Who is tirzepatide for?
Tirzepatide is generally appropriate for adults age 18 and older who meet medical criteria similar to semaglutide (BMI 27+ with weight-related conditions, or BMI 30+). Some patients prefer tirzepatide for its potentially stronger weight loss effect, particularly those who have tried semaglutide without sufficient results, or those who want to maximize weight loss in a shorter timeframe.
Like semaglutide, tirzepatide is not appropriate for people with a personal or family history of medullary thyroid carcinoma, MEN 2 syndrome, severe gastrointestinal disease, current pregnancy, active eating disorders, history of pancreatitis, or severe kidney disease.
What dose will I take?
Tirzepatide is typically started at 2.5 mg per week and titrated upward over time, with maintenance doses ranging from 5 mg to 15 mg per week depending on response and tolerance. As with semaglutide, dose increases at Trim Today never change your price — one flat rate covers any dose throughout your program.
What are the side effects of tirzepatide?
Side effect profile is similar to semaglutide and primarily gastrointestinal — nausea, constipation, diarrhea, occasional vomiting, and reduced appetite. Side effects tend to be most pronounced during initial dose titration and improve over time. Some patients find tirzepatide’s GI effects slightly more pronounced due to the stronger mechanism, particularly in the first few weeks. Anti-nausea support via Zofran is available as an add-on.
As with all GLP-1 medications, more serious but rare risks include pancreatitis, gallbladder problems, and kidney issues. Severe or persistent symptoms should be reported to your provider immediately.

Add-on treatments for your GLP-1 journey
Beyond the primary GLP-1 medications, Trim Today offers a curated set of supportive therapies that complement your weight loss plan, support cellular health, or address specific side effects. All are available as add-ons to any plan and require provider review and prescription.
Zofran
$29Generic: Ondansetron · 12-pill supply
Anti-nausea support
Zofran (ondansetron) is an FDA-approved anti-nausea medication that blocks serotonin receptors involved in the vomiting reflex. It’s the most commonly requested add-on for new GLP-1 patients, since nausea is the most common side effect during the first weeks of semaglutide or tirzepatide treatment.
Patients typically take Zofran on an as-needed basis when nausea occurs, with effects beginning within 30-60 minutes. Most people only need it during the first few weeks after starting GLP-1 or after a dose increase. Twelve pills provide enough for most patients to manage occasional nausea throughout their titration period.
Metformin
$59Generic: Metformin HCl · 30-pill supply
Metabolic support
Metformin is one of the most studied medications in the world, originally developed for type 2 diabetes. It works primarily by reducing glucose production in the liver and improving insulin sensitivity in the body’s tissues. When combined with a GLP-1 medication like semaglutide or tirzepatide, metformin can provide complementary metabolic benefits and may enhance weight loss results in some patients.
Metformin is often prescribed alongside GLP-1 therapy for patients with insulin resistance, pre-diabetes, polycystic ovary syndrome (PCOS), or metabolic syndrome. Some patients also use it as a standalone weight management tool, though GLP-1 medications are generally more effective for that primary purpose.
NAD+ Injection
$169Nicotinamide Adenine Dinucleotide
Cellular energy & recovery
NAD+ (nicotinamide adenine dinucleotide) is a coenzyme found in every living cell in your body. It plays a critical role in cellular energy production, DNA repair, and the regulation of metabolic pathways. NAD+ levels naturally decline with age, and supplementation has become a popular wellness intervention among patients seeking to support energy, mental clarity, and overall cellular function.
At Trim Today, NAD+ is offered as a compounded injection for direct administration. Many patients report improved energy levels, better mental focus, and faster recovery from physical exertion. NAD+ supplementation is often pursued by patients in middle adulthood or those experiencing fatigue, brain fog, or recovery issues alongside weight loss.
Lipo-C Injection
$99Methionine, Inositol, Choline + B12
Lipotropic fat metabolism support
Lipo-C is a compounded lipotropic injection that combines methionine, inositol, choline (collectively known as “MIC”), and vitamin B12. These compounds work together to support the body’s natural fat metabolism processes. Choline helps the liver break down and transport fats. Inositol supports lipid metabolism and may help regulate insulin signaling. Methionine assists with fat breakdown in the liver. B12 supports energy metabolism and red blood cell production.
Lipo-C injections are typically administered weekly as a complement to a comprehensive weight loss plan that includes GLP-1 medication, diet adjustments, and exercise. Many patients use Lipo-C throughout their weight loss journey to support fat metabolism and maintain energy levels.
Glutathione Injection
$89Reduced L-Glutathione (GSH)
Master antioxidant
Glutathione is often called the body’s “master antioxidant” because of its central role in neutralizing free radicals, supporting liver detoxification pathways, and maintaining cellular health. It’s a tripeptide produced naturally by the body from three amino acids (glutamate, cysteine, and glycine), but levels can decline due to age, stress, illness, environmental toxins, and certain medications.
Glutathione injections are pursued by patients interested in supporting immune function, detoxification, skin brightening, and overall cellular health. Some patients combine glutathione with other supportive therapies for comprehensive wellness during weight loss.
Sermorelin
$199Growth Hormone-Releasing Hormone analog
Growth hormone & body composition
Sermorelin is a synthetic peptide that mimics growth hormone-releasing hormone (GHRH), the natural signal your hypothalamus uses to tell your pituitary gland to release growth hormone. Unlike direct growth hormone supplementation, sermorelin works by stimulating your body’s own production through normal feedback mechanisms, which many practitioners consider a safer and more sustainable approach.
Adult growth hormone levels naturally decline with age, contributing to changes in body composition (more fat, less muscle), recovery time, sleep quality, and energy levels. Sermorelin therapy is pursued by adults seeking to support lean muscle mass, improve recovery, enhance sleep quality, and complement their weight loss results with improvements in body composition.

Services questions
Can I combine multiple treatments?
Yes. Most Trim Today patients combine a primary GLP-1 medication (semaglutide or tirzepatide) with one or more supportive therapies. Common combinations include semaglutide plus Zofran for nausea support during titration, tirzepatide plus Lipo-C for added fat metabolism support, and any GLP-1 plus Metformin for patients with insulin resistance. Your provider will help select which combinations are medically appropriate for you.
Do I need a separate prescription for each add-on?
Each medication requires its own provider review and prescription, but the process is integrated. When you complete intake, you indicate which treatments you’re interested in. Your provider reviews everything together and prescribes whatever is appropriate. Refills and management are unified through your single care team.
Can I add a supportive therapy after I’ve already started a GLP-1 plan?
Yes. You can add supportive therapies at any point during your program. Just contact your care team via SMS at 727-228-1025 or email support@trytrimtoday.com and we’ll arrange provider review and add the therapy to your shipments.
Are the supportive therapies FDA-approved?
Zofran (ondansetron) and Metformin are FDA-approved medications. NAD+, Lipo-C, and Glutathione are compounded wellness products and are not separately FDA-approved as drugs. Sermorelin is a compounded prescription peptide. All compounded medications at Trim Today are prepared by licensed 503A compounding pharmacies under FDA-regulated standards. Individual results vary, and these products are used as part of a comprehensive wellness plan rather than as standalone medical treatments.
What if I want a therapy that isn’t listed on this page?
